Findings
On or about February 25, 2021, Alleged Perpetrator 2 (AP2) emotionally abused Alleged Victim (AV) by using an intimidating posture while in conversation with AV which resulted in unreasonable emotional discomfort, triggering past trauma, causing emotional harm to AV. AP2's actions is considered Verbal/Emotional Abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
