Findings
The Alleged Victim (AV) has an unsteady gait, experiences dizziness, and has a history of falls. AV fell on February 10th and 11th, 2021 and again on February 21, 2021, suffering significant bruising to his/her left hip and buttock. AV's care plan does not address his/her falls. The facility failed to properly care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
