Findings
Alleged Victim (AV) relies on the facility to administer his/her medications. On or about March 2, 2021, Alleged Perpetrator 2 (AP2) was administering AV’s medication within the time frame window. AP2 documented this in the narcotic book but did not enter them in the Medication Administration Record (MAR). AV was later administered a second dosage since his/her medication where not signed for in the MAR. AV reported feeling dizzy. AP2 failed to provide a safe medication administration system, which is neglect of care and constitutes abuse. The facility failed to provide a safe medication administration system, which is a violation of Oregon Administrative Rules.
