Findings
The Alleged Victim (AV) has a history of falls and was placed on 30 minute checks to alleviate his/her falls. On or about March 17, 2021, AV suffered a fall and it was determined that staff were not following the care plan to check AV every 30 minutes. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
