Findings
On or about March 9, 2021, Alleged Victim (AV) suffered an unwitnessed fall, was sent to the hospital for treatment, and was diagnosed with a fracture to his/her left humerus and bruising to his/her back, face, and left arm. An investigation determined that facility failed to ensure AV's bed alarm was in place while AV was sleeping in his/her bed which resulted in AV exiting his/her bed without staff assistance. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
