Findings
On or about March 14, 2021, Alleged Victim (AV) was found on the floor by Alleged Perpetrator 2 (AP2) and Alleged Perpetrator 3 (AP3). AP2 and AP3 than proceeded to transfer AV prior to an evaluation by nurse or medication technician. AV's care plan states that AV is not to be moved if AV suffers a fall. AP2 and AP3 failed to follow AV's care plan which resulted in AV suffering a substantial injury to his/her right calf. AP2's and AP3's actions are considered neglect of care which constitutes abuse. The facility failed to provide AV a safe environment which is a violation of Oregon Administrative Rules.
