Findings
The facility failed to provide appropriate services according to the Alleged Victim's (AV) needs and care plan. The failure resulted in AV being found in soaked briefs and go hours without his/her meal assistance, causing unreasonable discomfort, a loss of personal dignity and risk of serious harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
