Findings
Alleged Victim (AV) is bedbound and relies on facility for all his/her care. AV is care planned to be checked on eight (8) times per shift. Documentation review and interviews determined AV was not getting checked on eight (8) times per shift as care planned. On or about March 4, 2021, AV was found with a small open area on his/her left bottom cheek. The facility failed to ensure care plans were followed which is a violation of resident rights, is neglect of care which constitutes abuse.
