Findings
In early January 2021, the Alleged Victim (AV) returned back to the facility after surgery and a stay at a nursing facility. Upon AV's return, the facility was responsible for ensuring that AV was transported to his/her medical appointments for follow up after surgery. The facility made the transportation appointments, however, did not follow through to ensure that AV got to his/her appointments as scheduled. AV has lost range of motion due to not going to these appointments and remains non-weight bearing. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
