Findings
The facility failed to properly care plan for Alleged Victim's (AV) fall risk. An investigation determined that AV suffered multiple unwitnessed falls with injuries with some of the injuries being head injuries. The facility failed to put interventions into place to prevent AV from falling which is a violation of resident rights, is considered neglect of care and constitutes abuse.
