Findings
On or about March 15, 2021, the Alleged Victim (AV) was found to have two bruises on his/her legs. AV is to have geri sleeves placed on his/her legs to reduce the risk of injury to his/her legs as AV tends to self transfer and harm him/herself. On this day, AV was found to not have the geri sleeves in placed as directed, leading to the bruises found. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
