Findings
On or about March 17, 2021, the Alleged Victim (AV) was found to have a skin tear on his/her legs. AV is to have geri sleeves placed on his/her legs to reduce the risk of injury to his/her legs as AV tends to self transfer and harm him/herself. On this day, AV was found to not have the geri sleeves in placed as directed, leading to the skin tears found. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
