Findings
On or about December 18, 2020, facility staff served Alleged Victim (AV) hot cocoa that was hot enough to cause serious burns when AV spilled the cocoa on his/her legs and groin area. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
