Findings
On or about March 15, 2021, Alleged Victim (AV) was found in his/her room with his/her pendant/lanyard wrapped around AV's trapeze on his/her bed and neck. An investigation determined that facility failed to intervene and/or provide interventions regarding AV's suicidal ideations and behaviors. The facility's failure to properly care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
