Findings
On or about December 20, 2020, the facility failed to provide a safe environment for Alleged Victim (AV) when the facility served hot tea to AV that was hot enough to cause a serious burn when AV spilled the hot tea on his/her right leg. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
