Findings
On or about March 23, 2021, Alleged Perpetrator 2 (AP2) used physical force against Alleged Victim (AV) when AP2 slapped AV across the face while in the role as AV's caregiver. AP2's actions is considered physical abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
