The facility failed to appropriately update the Alleged Victim's care plan to reflect his/her need for Geri sleeves. The failure resulted in AV receiving a skin tear on his/her arm and causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
