Findings
The facility failed to provide a safe environment and properly escort Witness 1 (W1) after exhibiting known aggressive behaviors. The failure resulted in W1 entering into a physical altercation with the Alleged Victim and causing him/her unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
