Findings
The facility failed to provide appropriate services, documentation and medical attention when the Alleged Victim experienced an injury to his/her ear. The failure resulted in AV having a permanent gap in his/her earlobe, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
