Findings
Alleged Victims (AV) relies on the facility for his/her care. On or about March 22, 2021, Alleged Perpetrator 2 (AP2) was assisting AV. AP2 became frustrated and referred to AV in a derogatory manner in AV’s presence. AP2's action are considered verbal abuse. The facility failed to protect AV from verbal abuse, which is a violation of Oregon Administrative Rules.
