Findings
Alleged Victim (AV) had a known history of falls. On or about March 27, 2021, AV attempted to self-transfer, lost his/her balance, and fell resulting in a head injury. AV was sent to the hospital and received eight (8) staples. The facility failed to properly plan care around AV’s known history of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
