Findings
The facility and AP2 failed to provide AV with the basic care, services, and staffing necessary to maintain his/her health and safety. The failure resulted in AV experiencing unreasonable discomfort and a loss of personal dignity, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
