Findings
On or about March 30, 2021, the Alleged Victim (AV) and Witness #1(W1) were involved in an altercation where W1 choked AV. AV and W1 had previous altercations that day. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
