Findings
The facility failed to provide a safe environment for the Alleged Victim (AV) or to follow his/her care plan to check that the pressure pad alarm was on. AV had an unwitnessed fall resulting in a head laceration requiring stitches. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
