Findings
Alleged Victim (AV) relies on the facility for his/her care. AVs care plan dated October 27, 2020 indicates AV can independently transfer and ambulate. AV has had six (6) falls from February 02, 2021 to April 08, 2021. The facility failed to plan care to mitigate the risk of injury due to falls which is a violation of residents rights is neglect of care and constitutes abuse as defined in OAR 411-020-0002(1)(b)(A)(i)
