Findings
On or about April 6, 2021, the Alleged Victim (AV) was discovered on the floor of his/her room from a fall, sustaining a bruise. AV had a bed cane installed at the facility, which was placed incorrectly causing the fall. The facility had no knowledge of the bed cane being placed, however, the facility is responsible to provide a safe environment. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
