Findings
The facility failed to follow Alleged Victim's (AV) care plan which resulted in AV developing a rash due to the facility staff not assisting AV with his/her toileting needs as outlined in his/her care plan. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
