Findings
The facility failed to provide a safe environment for Alleged Victim (AV) by using wrongful restraint against AV during showering. An investigation determined that the facility did not have doctor's orders to use restraint to get AV to shower when he/she was combative. The facility's failure to obtain an assessment and physician's prescription for restraining AV during showers is a violation of resident rights, is considered neglect of care and constitutes abuse.
