Findings
Alleged Perpetrator #2 (AP2) gave discontinued medication to the Alleged Victim (AV), and AV had already received the medication that day. AV experienced no negative outcome. The Facility failed to provide a safe medication administration system which is a violation of Oregon Administrative Rules.
