Findings
On or about May 31, 2021, the Alleged Victim (AV) suffered a change in condition where he/she required staff to assist with feeding. AV's care plan was not changed until April 20, 2021 to include assistance with feeding, leaving AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
