Findings
On or about April 18, 2021, Alleged Perpetrator 2 (AP2) grabbed onto Alleged Victim's (AV) left wrist and held him/her down when AV became combative while refusing care which resulted in AV getting a quarter size bruise on the top of his/her left wrist. AP2's actions is considered physical abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
