Findings
On or about April 19, 2021, Alleged Victim (AV) fell in his/her bathroom suffering a Right Subdural Hematoma. An investigation determined that AV suffered seven (7) falls since March 12, 2021. The facility failed to properly plan care for AV for his/her fall risk which is a violation of resident rights, is considered neglect of care and constitutes abuse.
