Findings
On or about April 19, 2021, the Alleged Victim (AV) had a thumb nail that was bending back and bothering AV. Alleged Perpetrator #2 (AP2) ripped the nail from AV's thumb causing unreasonable discomfort. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The Allegation for AP2 for physical abuse was founded to be inconclusive. The facility's failure is a violation of Oregon Administrative Rules.
