Findings
On or about April 6, 2021, the Alleged Victim (AV) was on the facility bus going on an outing. When the bus arrived at the location, the driver was placing the ramp down on the bus. AV stood up and when the bus was placed into park, the bus jerked and AV fell to the floor, suffering pain and bruising. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
