Findings
Alleged Victim (AV) relies on the facility for his/her care. On or about April 21, 2021, family found AV laying in soaked bedding, and the bedding had not been changed during a week’s time. This happened on more than one occasion. The facility failed to care plan in accordance to AV’s toileting needs, which is a violation of resident rights, is neglect of care and constitutes abuse.
