- On or about April 26, 2021, Alleged Victim (AV) was found by facility staff to have an injury of unknown origin on his/her right side along his/her ribs. Three days later, AV was observed by facility staff to not be at his/her baseline for behavior, appeared to be in pain, and had swelling to his/her abdomen. AV was taken to the hospital for treatment and was diagnosed with eight (8) fractured ribs on his/her right side. An investigation determined that the facility failed to provide a safe environment for AV by failing to get AV appropriate medical attention for approximately three days after finding an injury of unknown origin and for failing to put effective interventions in place to address AV's frequent falls. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
