Findings
Alleged Victim (AV) relies on the facility for his/her care. Between March 5, 2021, and April 29, 2021, AV was exhibiting increased wandering within the facility and experiencing a rapid cognitive decline. On or about April 29, 2021, AV was found outside the assisted living facility. The facility failed to care plan for AV's change of condition, which is a violation of resident rights, is neglect of care which constitutes abuse.
