- Alleged Victim (AV) relies on the facility for his/her care. From February 28, through April 18, 2021, AV sustained six (6) falls. On or about April 18, 2021, AV sustained two (2) falls in the same day. AV’s first fall resulted in a laceration to AV’s left knuckle and the second fall resulted in AV been sent to the emergency room (ER) with a laceration to his/her eyebrow, skin tears to his/her cheek and hand and a black eye. The facility failed to care plan appropriately and implement interventions to mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
