- Alleged Victim (AV) relies on the facility for his/her care. AV was care planned to have assistance while ambulating, safety checks, encouragement to use his/her walker and non skid socks. On or about April 30, 2021 AV had an unwitnessed fall while independently ambulating. The facility failed to follow AV’s care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
