Findings
On or about May 3, 2021, Alleged Victim (AV) and Witness 1 (W1) had a resident-to-resident altercation in which AV sustained a bump to the back of his/her head and bruising developed in AV's rib area the following day. The facility failed to provide a safe environment for AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
