The Alleged Victim (AV) eloped from the facility on May 4, 2021, unknown to staff at the facility. AV was found by police 6 miles from the facility, which placed AV at risk for harm. The facility failed to have appropriate interventions and failed to provide a safe environment for AV. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
