Findings: Alleged Victim (AV) relies on facility for his/her care. Witness 1 (W1) has a history of resident-to-resident altercations. On or about May 3, 2021, W1 and AV got into an altercation when AV attempted to stop another resident-to-resident incident involving W1. The facility failed to provide a safe environment which is a violation of resident rights, is neglect of care and constitutes abuse.
