- Alleged Victim (AV) relies on the facility for his/her care. AV had sustained two falls on or about April 22nd and 28th, 2021, resulting in skin tear, head injury, facial bruising, bruising at back and shoulder. On or about April 14, 2021, prior to moving into the facility, hospital records indicated that AV had chronic edema in legs and was to wear compressions stocking per attending physician’s advice. Staff were not using compression stocking on AV’s legs and AV’s legs were weeping. The facility admitted to not having the right size compression stockings that fit AV and had not received new stocking. The facility did not receive the proper stockings for AV until May 18, 2021. The facility failed to provide service to AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
