- The Alleged Perpetrator #2 (AP2) began employment on or about April 6, 2021. On or about April 14, 2021, AP2 destroyed controlled medications without a second staff present, and had a Non-licensed staff sign the unwitnessed destruction of the controlled medications, which is against facility policy. AP2’s actions are a violation of resident rights, are considered neglect of care and constitute financial abuse. The facility failed to ensure AP2 had signed off on proper training and provide a safe medication administration system, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
