- The Alleged Perpetrator #2 (AP2) began employment on April 5, 2021. On or about April 15, 2021, AP2 destroyed controlled medications without a second staff present, which is against facility policy. On or about April 26, 2021, AP2 transferred controlled medications to a new page, however, 4 tablets of controlled substance were unaccounted for. On or about April 29, 2021, AP2 again destroyed controlled medications without a second staff present. AP2’s actions are a violation of resident rights, are considered neglect of care and constitute financial abuse. The facility failed to ensure AP2 had signed off on proper training and provide a safe medication administration system, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
