- On or about April 27, 2021, it was discovered that the Alleged Victim (AV) had a large wound on his/her heel. Staff were to do skin checks during sponge baths to ensure the integrity of AV's skin. Staff did not notice AV's wound until it was brought to the attention of the nurse by AV and at that time, AV's wound had reached 2.5 x 4 x .2 cm. The facility failed to intervene when AV's condition changed, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
