Findings
The facility failed to provide a safe environment for Alleged Victim (AV) which resulted in AV being slapped five or six times by Witness 1 (W1) when W1 approached AV in the dining room. The facility's failure to protect AV from W1's aggressive behaviors is a violation of resident rights, is considered neglect of care and constitutes abuse.
