Findings
On or about May 12, 2021, Witness #1 (W1) was observed by staff slapping the Alleged Victim (AV) who was in bed at the time. AV attempted to get away from W1 and fell causing an abrasion to his/her knee. When staff re-directed W1, he/she slapped AV on the back while exiting the room. W1 is known to have behaviors and agitation. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
