Findings
On or about May 13, 2021, Witness #1 (W1) entered the Alleged Victim's (AV) room. AV yelled and W1 slapped AV in the face. AV did not sustain any injuries, however, AV was placed at risk for harm. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
