Findings
On or about May 15, 2021, the Alleged Victim (AV) had water thrown in his/her face by Witness #1 (W1). W1 is to be monitored by staff and redirected when W1 demonstrates behaviors that could lead to negative interactions. The facility's failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
